Fifth Grade Parent Satisfaction Survey

Dear Parents/Guardians:

 

Invitation to Participate: Throughout the school year we have asked for your feedback on how your children have responded to the piloted 5/6 Health and Wellness Curriculum.  Now as we near the end of the year we are asking for your help once again to evaluate the overall quality of the curriculum to ensure that we have reached our goals.  I know that this is a busy time of year for you, but I hope that you will take just a little time to participate in this brief web survey.

Purpose:  Questions arose while developing the curriculum about what topics were most important to be included in a core wellness curriculum and just how extensive or far reaching such a curriculum should be in order to be considered adequate.  Additionally, questions also presented themselves about the depth to which any particular topic needed to be addressed in terms of it being age appropriate and what indicators we might use to measure whether students are actually learning the material in a manner that promotes their own lifelong health and wellbeing.

           

In order to answer these questions we reviewed several health, physical education, and wellness programs with our target population of the fifth and sixth grades in mind.  We consulted with psychologists, pediatricians, physical education specialists, nurses, and experts in other fields such as adolescent sexuality and stress management to ascertain what they perceive to be part of a core wellness curriculum for this age range and to what degree the material should be presented. 

           

As a result of our research we identified a core wellness program for the fifth and sixth grade that represented the results of our initial investigations.  We identified six Domains that, in turn, are each constituted by a cluster of important topics: Health Promotion and Disease Prevention; Personal and Interpersonal Life Coping Skills, Tobacco, Alcohol and other Drugs, Human Growth/Development and Sexuality; Fitness; and Safety.

           

Confidentiality: Your answers will be completely confidential. Moreover, the results of the survey will be reported in a summary format, so again no one will link you to your responses. Text comments will be reported verbatim, so please do not provide identifying information in your text comments.
