If you are receiving this email it is because the Health Office has yet to receive the Dental and Over the Counter (OTC) Medication Forms for your child(ren). The dental forms are required by DC health regulations and although the OTC form is not mandatory, it is necessary if your child (ren) require(s) any medications to be administered from the Health Office, i.e. Tylenol, Neosporin, etc. Please note that the nurse <span style="text-decoration: underline;">will not</span> be able to administer any OTC medications without this form, it must be signed by you <span style="text-decoration: underline;">and</span> their pediatrician. The necessary forms have been attached. You can fax to 202-537-2438, scan and email to me, hand deliver to my office or mail.<br />
