Dear Friends, If you are receiving this email it is because the Health Office has yet to receive the Over the Counter (OTC) Medication Forms for your child(ren). Although the OTC form is not mandatory, it is necessary if your child (ren) require(s) any medications to be administered from the Health Office, i.e. Tylenol, Neosporin, etc. Please note that the nurse will not be able to administer any OTC medications without this form, it must be signed by you and their pediatrician. The necessary form has been attached. If you choose to opt out, please disregard. You can fax to 202-537-2438, scan and email to me, hand deliver to my office, or mail. If you have already made contact with me regarding these forms, please do not respond to this message.
