Dear Parents,

I am writing another note as we have not received your child/children's
health forms. As you know from several notices, we are required to have
a current SFS Physician's Report and an up-to-date immunization record
on file before students attend class at the beginning of the school
year. Please get in contact with me before Tuesday 9/7 so I have
information about the status of your family's health forms. I will be
happy to work with you to complete this requirement if you contact me by
email or phone this weekend.

If you do not contact me, your child may not attend school on Tuesday
and I certainly want to help you prevent that from happening as we are
striving to start the new school year on a calm and happy note.
