<p><span style="font-size:12.0pt"><span style="font-family:arial,helvetica,sans-serif">I am writing to let you know that I have not yet received the <u>second signature</u> for your child’s <strong>Consent to Treat Form. </strong>This form is required for your student to attend classes on the first day of school. In order to successfully complete this requirement, an electronic signature from all parents/guardians must be submitted. This means that each parent/guardian will need to <strong><em>separately sign in with their own login credentials</em></strong> and electronically sign the document. <strong>You will not be able to submit 2 signatures under the same login</strong>. Enter your email address, as the username, and your password to login to the Parent Portal and click on the “<em>Health Forms</em>” link to access the form under your child’s account. </span></span></p>

<p><span style="font-size:12.0pt"><span style="font-family:arial,helvetica,sans-serif">Please note that if you have already submitted your signatures, your Magnus account will indicate completion under each parent/guardian’s own individual login credentials. You must check under both names in order to verify successful completion.</span></span></p>
