Hi Joan,


I hope you’re well and gearing up for a happy holiday season! As the year
comes to a close, it seems we are missing Hunter’s Conflict of Interest
Forms for his 2017 Board term with USGLC. The USGLC is legally required to
circulate the conflict of interest form each year to our Board of
Directors. Attached please find the form for Hunter’s review and signature
page to return back to the USGLC.


If you could please send back before the end of the year and please let me
know if you have any questions!
