40th Anniversary Flame of Hope Recognition
CALL FOR NOMINATIONS
Deadline October 10, 2017

Celebrate the Chapter’s 40th Anniversary honoring Ellyn Phillips by recognizing other individuals who have made a positive impact in the fight against ALS. Your nominations in the categories listed below exemplify extraordinary persons who every day bring hope and hope to ALS families. Those selected will be presented with a Flame of Hope at the Chapter’s 40th Anniversary Gala on Thursday, November 9 at Vie in Philadelphia.

The nominating committee of Chapter Board and Staff will review your nominees and score them from a set scale. All recipients must reside in the Chapter’s service area of PA, NJ and DE. Chapter Staff and Board are not eligible for recognition while serving in their position for the Chapter. Thanks so very much!
Arthur and Lea Powell Care Services
To an individual who exemplifies the Chapter’s mission of helping ALS families live their lives to the fullest. Examples include Visiting Volunteers, respite providers, educators on care and safety for families living with the disease, and others who have gone above and beyond to provide compassionate care to those affected by ALS.
Advocacy & Public Awareness
To an individual who has advanced the Chapter’s mission through public policy and awareness at the State and/or Federal Levels: Examples include someone who attends Advocacy Days, promotes advocacy through correspondence and visits to their elected officials, boosts the profile of the Chapter through media, or raises awareness by participating in Chapter fundraising events.
Innovation & Research
To an individual who has contributed to research initiatives to improve ALS treatment and move closer to a cause and cure for the disease. Examples include scientists studying the disease or patients or family members who participate in clinical trials and/or research studies, fundraisers/events designating dollars to research.
