Dear US Families,

If you are receiving this email it is to alert you that you have at least one (if not more) health form(s) to complete and upload to the portal. As has been communicated, we are not legally permitted to allow students who do not have all forms completed to attend school activities and classes, including Orientation for ninth graders on Thursday, August 30th . If students arrive for scheduled school activities and classes without all forms completed, we will need to send them to the US Dean’s Office until the forms are submitted and approved. It is very important that we have all forms per DC law, so thank you for your prompt attention to this matter.

The good news is that there is still time to get these forms completed. In order to avoid surprises, each division is contacting families with missing information to remind them of the need to complete the forms as a one-time follow-up to other communications. This is the only reminder that you will receive from Upper School. Please monitor the Magnus Health portal for the status of your forms. Please also be aware that the school is unable to review and approve forms submitted over the Labor Day long weekend. Therefore, in order for your children to be able to join their classes, all forms must be uploaded and approved by Thursday, August 30th.

There are two forms exempt from this guideline. The first is the dental form, if you have an appointment scheduled. Please email Anneke Collins and Jasmin Whitfield (copied on this email), to let us know the date of the appointment. The other form is the OTC form. While we encourage you to complete this, your child will be able to attend classes if it is not completed.
