For Parents of Minors: Letter of Consent for Travel of a Minor
If your child will be a minor at time of travel, you are required to complete and submit a notarized copy, or copies, of the Letter of Consent for Travel of a Minor. This form is required for any minor traveling with the School on an international trip. This is a new form required because of recent legislation against child abduction and human trafficking. Attached is a template that we have pre-filled with information specific to your trip. All custodial parents will need to submit a copy of this form.  Families may use the same form or each parent may submit a different form. There are three notaries available from 8am-4pm at SFS in Zartman House for your convenience, listed below. Please call in advance to schedule time with one of them. You must sign the form in front of a notary, so please bring the form unsigned when you meet with the notary. We ask that all forms be submitted to the Global Programs Office (attn.: Shoshanna Sumka).
