Hi Hunter and Kathleen,

Sidwell Friends School is currently asking all parents/guardians to
complete their emergency contact information online. You should have
received an email sometime this morning. Due to security reasons, the
children will not be able to complete this information online so I have
attached electronic copies of the same information that is being
requested of all households. Please complete the attached information
and you can hand deliver or mail directly to me in the Health Office by
no later than June 1st or report cards will be held until the forms have
been received by the School.

Health forms for the next school year are also due by July 15th. Please
have the physician complete all forms attached, one form per child. The
HPV opt out form will only be needed if you decide not to have Maisy
receive this vaccination.
