Photography/Video Permission, Online and Print Directories, and Media Release

I understand that the default below is for "I do authorize" which gives Sidwell Friends School permission to release Student or Parent information in the print and online directories or other media outlets and/or use photographs of the Student.

If I do not authorize this release, I will select "I do not authorize" and complete the required form provided below.

I do authorize Sidwell Friends School permission to:

1. Use, in whole or in part, photographs, videos, written extractions, or voice recordings of the student named here for publication on the School website, or in School marketing materials and other print and digital communications.
2. Publish contact information (residential and e-mail addresses and phone numbers) for this student and his or her parent(s)/guardian(s) in the printed student directory or the password-protected online directory, both of which are made available to School families and employees.
3. Release this student's name as part of a School athletic team roster to media outlets, if applicable.

I do not authorize one or more of the options listed above for the Student and will complete the required opt-out form provided below.
