In keeping with Sidwell Friends’ environmental commitment, this information will be collected online. One parent can update and submit the online forms for the entire household. Please complete the online forms no later than June 1st. Instructions: Either select this link "Update Household/Student Information" or go to the Sidwell Friends School website: www.sidwell.edu , select "Parents" on the upper right and then select “Update Household/Student Information” found on the left side, below the "Parent Portal" banner. After logging-in, follow the directions to update and complete the forms online. All households, including new and returning families, need to confirm all contact information and complete the forms. In addition, a custodial parent/guardian will need to complete a health questionnaire, fill in the health insurance information and an emergency contact for each child. Information gathered will help our school community provide a safe and supportive learning environment for your child(ren). For returning students, June report cards will be held until the online forms have been submitted. In addition, you will not be able to access the forms if you have not completed a re-enrollment contract for 2010-11.
