Per our Bylaws, this form is an annual requirement for the members of the
Board of Directors and the Officers of WFP USA. The document includes a
statement of our Conflicts Policy, questions for you to answer and a copy
of the full By-Law regarding Conflicts of Interest for your reference.


Please complete the form and return it to me *by December 31* via e-maiI or
regular mail. If you return an electronic copy, it is acceptable for you
to use an electronic signature on the document. Thank you for your prompt
