If the student’s birthday or appointment falls between September and December, a parent may request a note from the pediatrician stating the student is “cleared to participate in sports/attend school” and include the date the new physical exam is scheduled. - Upload this note to the Magnus Portal under the Physical Evaluation Form link. In addition, you are still required to submit the Physical Evaluation Form once the appointment has occurred. - No student will be allowed to participate in any school-related activities including preseason sports, academic classes, rehearsals, etc. if the School does not have the appropriate form on file with Magnus. D.C. Department of Health Immunization Requirements Please note the latest immunization schedule. Please consult with your pediatrician to discuss options.
