Tuesday - May 8, 2012 From the School This Week's Query How is Simplicity measured in a community? Notes from Sally Assembly: “Bag It” Parents of all current sixth graders are invited to the Transition Meeting on Wednesday night at 7:30 in the Drama Studio. Please come at 6:45 to meet and help welcome families who are new to the grade. There is a transition meeting next Wednesday, May 16th for families new to 6th grade only. Current parents need not attend, although we know that some will be coming for the welcome reception! All 7th and 8th graders will be participating in Service Activities this Thursday. Please be on the lookout for permission slips and return them promptly. Please plan to bring your child to the Dance Marathon THIS FRIDAY! This is really going to be a special, memorable event filled with terrific food, games, activities, music, Pinkberry yogurt, and of course dancing! Young patients from Children's National Medical Center will be there to join in the fun and inspire our students. This is a rare opportunity for all MS students to come together for a wonderful service opportunity and show themselves what they can do together to help others who need their help. The entire Middle School will be engaged in service activities on the morning of May 24th. One of the activities we will be doing that day is packaging toiletries for donation. If you have extras around the house, please send them in before May 24th! There is a basket in the office. Parents of 8th graders: you have received an email about PE/Athletics sign ups for next year. Registration is open from May 15th – 22nd. Please contact the PE office if you have questions. Current 6th graders will sign up for sports during their regular PE times in the next week or two. First Grade teachers Virginia Singer and Ellie Lash are retiring at the end of the school year after 60+ years of combined service. Their current first graders are putting together special scrapbooks for each of them. They would like to make the scrapbooks more meaningful with notes and thoughts from former students. If your child was in Mrs. Singer and/or Mrs. Lash's class, encourage them to take a moment to write a nice note to each of them. They can include favorite memories from first grade and best wishes. Blank note cards for students are available in the Middle School Office and may be returned there when completed. All health forms are on the Magnus Health portal. If you have not received your login information for Magnus Health, you must contact their support desk as soon as possible. Call toll free 1-877-461-6831. Back-to-School Health Form Guidelines for DC Campus Students Physical Evaluation Form: • All students must have a physical exam within the 2012 calendar year. If the student’s birthday or appointment falls between September and December, a parent may request a note from the pediatrician stating the student is “cleared to participate in sports/attend school” and include the date the new physical exam is scheduled. • Upload this note to the Magnus Portal under the Physical Evaluation Form link. In addition, you are still required to submit the Physical Evaluation Form once the appointment has occurred. • No student will be allowed to participate in any school-related activities including preseason sports, academic classes, rehearsals, etc. if the School does not have the appropriate form on file with Magnus. D.C. Department of Health Immunization Requirements: Please note the latest immunization schedule: click here. Please consult with your pediatrician to discuss options. If you choose to have your child(ren) opt out, please note that you are required to submit proof of exemption as follows: • For the HPV vaccine required for all girls entering 6th – 8th grade, make sure that you have completed the HPV opt out form posted on the Magnus Portal. • For all other immunizations, ensure the doctor has completed either Sections 2 or 3 on the 2nd page of the DC Health Form. • Or complete the Religious Exemption Form on the Magnus portal for all other religious or personal reasons. Proposed Change to PA By-laws The Parents Association (PA) is proposing a change to the PA by-laws. Changing the by-laws requires a written notice to all members of the Parents Association. Following this notification, the by-laws change may be enacted by a consensus of the PA All School Committee. The proposed change is to Article X: Nominations, Slate Selection, and Appointments and involves striking the phrase "who are not simultaneously serving in any other capacity on the All-School Committee" from the first sentence in Section 1.
