Jim,

Thank you for taking my call. I am glad we are able to offer a new discounted rate for Hunter.

The new agreement that we spoke about is as follows. We will charge a daily rate of $2,500.00 and we will ask for a five day deposit. In the event that Hunter does leave against treatment advise the 5 day deposit will be non refundable.

If Hunter needs to stay a bit longer we will charge the card on file daily.

Medication and the Dr. Fee are a separate charge. So we will need to collect a one time Dr. Fee of $1500.00 as well as any medication needed we will apply to the card.

Please let me know if this will work for you.

I have attached the CC Auth from below. If this is ok with you please fill out the form and email it back to me as soon as you are able. The amount to be charged will be $14,000.00. That is $2500 x 5 plus the $1500 Dr. Fee.
