Dear Summer Travelers,
Please refer to the corrected information below in regards to Medical forms in preparation for your summer trips.
Medical Forms
Although trip registration is online there are a few paper forms to complete concerning medication and medical management. These optional forms are attached. If you have already submitted these forms at the start of the school year and there is nothing new to add, they do not need to be resubmitted.  All physical evaluations must be up to date.
If your child is going to be taking Over-the-Counter (OTC) or prescription medicines during this trip, PLEASE READ CAREFULLY!
1        Prescription Medication Authorization Form: This form needs to be completed and signed by you and your child’s health care provider and should include any over-the-counter medicines (that are not already listed on the Over-the-Counter (OTC) form) as well as any prescription medicines that you would like the Trained Medication Aide to administer.  Any medicines that you are requesting to be administered must be listed on the form. OTC, prescription and homeopathic medicines are treated the same and must be listed on this form or they will not be administered.
2        Over -the–Counter (OTC) Medication Authorization Form: If you did not submit an OTC form in the beginning of the school year, we suggest that you complete one now.  Both a parent and doctor's signature are required on the OTC form.
3        Medical Action Plans: Including Asthma, Allergy, Diabetes or Seizure must be completed if there are medications associated with any of these diagnoses that need to be administered. These plans include action steps that help the Trained Medication Aide provide immediate and appropriate care for your child while away from home.
If you do not remember whether you have already completed any of these forms, you can check via Magnus Health through the parent portal by clicking on the link “Update Student Health Information”.
Next steps with any medications:
&#8226          All prescription and over-the-counter medicines to be administered must be turned in to the Health Services Office no later than two weeks before the trip leaves (earlier is even better).
&#8226          All medications must be in its original labeled pharmacy containers and placed in a Ziploc-like bag.
&#8226          The Health Services Team is required to review all of these medicines with applicable forms. A member of the Team then meets with trip leaders to review them and provide any necessary education and/or instruction before the trip.
&#8226          You do not need to provide any of the medicines that are listed on the OTC form as the Health Services Team gives us a supply.
&#8226          Students with asthma inhalers or Epi-pens should carry them at all times on their person, but you can provide extras in this Ziploc-like bag.
