Thank you for sending in the subscription form and for your support of
American Well.

To finalize this transaction, please have the following completed on behalf
of Rosemont:
1) The attached Accredited Investor Questionnaire (please send me a
copy once signed); and
2) Send a wire or a check in the amount of $50,040 to American Well
pursuant to the attached transfer instructions. If your bank charges a fee
for wiring funds, please adjust the amount of the wire to account for that
fee.

Following receipt of the questionnaire and the wire/check, we will issue
Rosemont a stock certificate and send back a countersigned version of the
signature page to the subscription agreement.
