Dear Friends, The deadline for DC health forms has now passed. If you are receiving this email it is because the School has yet to receive health forms for your child(ren). All forms must be received prior to orientation or preseason sports, whichever comes first. If forms are not received prior to this date, your child will not be allowed to attend classes or sports activities. The necessary forms have been attached.
