Good Afternoon,

This is just a friendly reminder that your child's Physical will expire on 8/31/17. You have a 30 day grace period to complete the physical form. Your child’s DC Immunization form, Consent to Treat Form, and Vital Health Record need to be completed as well. Your child’s name was on the Girls Soccer roster and in order for your child to participate in preseason the DC Immunization form, Consent to Treat Form and Vital Health Record need to be completed before she can practice. Again, the physical form has a 30 day grace period. Please login to your child's health tracker by signing into the Parent Portal and clicking on Update Student Health Information to review what is missing or required and or to submit the updated DC Universal Health Certificate and other paperwork.
