Hello Minimester Families! This is a friendly reminder that the 2nd payment for this trip is due on 2/15. A statement showing your current account balance can be found attached to this email. Payment can be submitted in 3 different ways: 1 – Do nothing! The credit card used for the initial deposit will be run on the dates listed above. If you would like to use an alternate form of payment please use one of the options listed below. 2 – Send a check into school before the date listed above attention to Whitney Gray 3 – Log into your existing account and process a payment with a new credit card. To access your account please use the Manage Your Account link found on the trips page. Medical Forms Reminder As a reminder, if your child is going to be taking Over-the-Counter (OTC) or prescription medicines during this trip, please following the instructions below. If you have already submitted these form at the start of the school year, and do not need to add anything, they do not need to be resubmitted Prescription Medication Authorization Form: This form needs to be completed and signed by you and your child’s doctor and should include any over-the-counter medicines (that are not already listed on the Over-the-Counter (OTC) form that you may have submitted in the beginning of the school year) that you would like us to be able to administer as well as any prescription medicines. Remember, it’s not uncommon for kids to get a stomach ailment or cold while traveling. Any medicines you would like us to administer must be listed on the form. If you did not submit an OTC form in the beginning of the school year, we suggest that you complete one now. Both a parent and doctor's signatures are required on the OTC form. If you do not remember whether you have already completed any of these forms, you can check via Magnus Health through the parent portal. Next Steps with any medications: All prescription and over-the-counter medicines you would like us to be able to administer must be turned in to Nurse Jasmin no later than two weeks before the trip leaves (earlier is even better). All medications must be in original pharmacy containers and placed in a Ziploc bag. Nurse Jasmin is required to review all of these medicines. She then meets with trip leaders to go over them before the trip. You do not need to provide any of the medicines that are listed on the OTC form as Nurse Jasmin gives us a supply. Students with asthma inhalers or Epi-pens should carry them at all times on their person, but you can provide extras in this Ziploc if that would be prudent. Please help out Nurse Jasmin by meeting this deadline. If you have any questions, feel free to contact her at (202)537-8130 or whitfieldj@sidwell.edu. Should you have any questions please feel free to contact me.
