Dear Friends,
This is a friendly reminder that the deadline for DC health forms has now passed.  If you are receiving this email it is because the School has yet to receive health forms for your child(ren).  Preseason officially begins August 23rd and if forms are not received by this date, your child will not be allowed to participate in sports activities.  If your child is not participating in preseason sports activities, the forms will need to be received prior to the first day of school (September 7th) or they will not be allowed to attend classes.  The necessary forms have been attached.
