Good afternoon, As of Today, Saturday, Maisy is still missing most of her paperwork for preseason. The second signature on her Consent to Treat, her Physical Evaluation form, and her Action Plans must be filled out before 8:00 am on Monday morning, if not, Maisy will not be able to participate in the first preseason practice. Please remember, both parents have their own login credentials and each parent needs to login to their account to add the signature. Please let me know if you have any questions.
