This is just a friendly reminder that your child’s Consent to Treat Form and Action plans need to be completed. Her name is listed on the Soccer roster and in order for her to participate in preseason these forms need to be completed before she can practice. Please note that this year the Consent to Treat Form needs both parents’ signatures and each parent will need to sign into their parent portal to complete this
form. Also, I wanted to note that her physical expires on 8/14/18 so she will be an updated physical form before preseason as well. Please let me know if you have any questions or if I can help in any other way.
