Thank you so much for the update. After her physical is completed we will still need the Consent to Treat Form sign by Hunter. Once that is completed Maisy will be cleared. Let me know if we can help with anything.

Today was the earliest they would take Maisy. She has an appointment at 11. As for the forms, I filled them all out earlier in the summer. I will check the forms again this morning.

As of Today, Saturday, Maisy is still missing most of her paperwork for preseason. The second signature on her Consent to Treat, her Physical Evaluation form, and her Action Plans must be filled out before 8:00 am on Monday morning, if not, Maisy will not be able to participate in the first preseason practice. Please remember, both parents have their own login credentials and each parent needs to login to their account to add the signature. Please let me know if you have any questions.
