<p><span style="font-size:11.0pt"><strong>Reminder</strong>: Please complete your health form requirements via the Parent Portal. <strong><em>If you have completed this process in its entirety, thank you so much and please disregard this notification.</em> </strong></span></p>

<p><span style="font-size:11.0pt">Health forms must be completed and submitted before preseason or before the first day of school (whichever comes first). Preseason begins August 16th for football and August 20th for all other sports. Students will not be able to attend any classes or participate in sports until all required health forms have been submitted and approved by Sidwell Friends School.</span></p>

<p><span style="font-size:11.0pt">You may access the health forms by signing into the Parent Portal and click on the Health Forms link located on the right side of the page. When you log in you will need to click on each child’s name to see what forms are required for their upcoming grade level. <em>Please note that the forms must be submitted online via Magnus Health. No paper health forms will be accepted at the School.</em></span></p>

<p><strong><span style="font-size:11.0pt">Annual Physical Exams</span></strong><span style="font-size:11.0pt">: All students are required to have an annual physical exam during the 12-month period immediately preceding the first day of the school year. A DC Universal Health Certificate must be completed and signed by a health care practitioner and submitted to the School via the Parent Portal by clicking on the <em>Health Forms</em> link. </span></p>

<p><span style="font-size:11.0pt">If you find that you will not be able to furnish the School with an updated Health Certificate by the expiration date of the last physical examination, you may have the health care practitioner write a note indicating that your child “is cleared to participate in physical and other school related activities until next physical exam scheduled on….” This note will suffice until that scheduled physical exam is completed. Once completed, the Health Certificate should be submitted via the Parent Portal by clicking on the <em>Health Forms</em> link.</span></p>

<p><strong><span style="font-size:11.0pt">Immunizations</span></strong><span style="font-size:11.0pt">: In accordance with DC law, all students must submit documentation that they have been vaccinated against specific illnesses before the school year begins. Tdap, Meningitis and HPV immunizations are required by the D.C. Department of Health for students 11 years and older entering 6th through 12th grades.</span></p>

<p><span style="font-size:11.0pt">No student will be allowed to attend School without a Certificate of Immunization unless the student has been exempted in accordance with law. In order for a student to be exempted from providing certification of immunization, parents must submit a completed religious or medical exemption form on an annual basis to Magnus Health, via the Parent Portal by clicking on the <em>Health Forms</em> link. </span></p>

<p><strong><span style="font-size:11.0pt">Consent to Treat Form:</span></strong></p>

<p><span style="font-size:11.0pt">Requires an electronic signature from all parents/guardians that the School has on file. For this document to be approved, each parent/guardian will need to login with their own login credentials and electronically sign the document. </span></p>

<p><strong><span style="font-size:11.0pt">Vital Health Record:</span></strong></p>

<p><span style="font-size:11.0pt">All (7) sections of this electronic health history needs to be individually opened, reviewed, and submitted for approval. Please remember to click "Submit" in each section to indicate the information has been reviewed and is up-to-date. </span></p>
